How do I get to Fauquier Health?

500 Hospital Drive
Warrenton, VA 20186

Who will pay for my stay?

Rehabilitation services are fully or partially covered by most health insurance plans. Most managed care insurance plans require a pre-authorization for rehabilitation services.

Am I at risk for a fall?
  1. Have you experienced a stroke, diabetes, osteoporosis, Parkinson's disease or other neurological problem that has affected your balance?
  2. Do you take medicine for two or more of the following diseases: heart disease, hypertension, arthritis, anxiety, depression, pain or thyroid?
  3. Have you fallen more than once in the past year?
  4. Do you use a walker or a wheelchair, or do you need assistance to get around (including holding on to furniture when indoors)?
  5. Do you have difficulty sitting down or rising from a seated or lying position?
  6. Do you feel dizzy or unsteady if you make sudden changes in movement, such as bending over, quickly turning while walking, lying down, looking up or quickly turning your head?
  7. Due to balancing difficulties, do you restrict or are you fearful of the following activities?
    • Taking a walk
    • Carrying a full plate across the room
    • Getting in and/or out of the car
    • Getting on or off the commode
    • Stepping off curbs
    • Climbing stairs
    • Walking on a crowded sidewalk

If you answered YES to any of the above questions, you could benefit from a consultation by a physical therapist who specializes in balance.

How can a physical therapist reduce my risk of falling?

A physical therapist can help you with:

  • Exercise: Learn how to improve your range of motion, strength and cardiovascular capabilities
  • Education: Learn how to make your environment safe and how to select proper footwear for different surfaces
  • Sensory-deficit awareness: Learn how to adjust or compensate for your losses, and learn energy-saving techniques to allow you to perform activities of daily living safely
  • Gait training: Improve your ability to walk with or without a device such as a cane, walker or crutches
  • Balance re-education: Learn how to compensate for poor balance to make you safer in your home environment
  • Postural re-education: Become aware of your posture problems and learn to perform the proper stretches and exercises that will make you safer while moving
  • Vestibular rehabilitation: A specialized exercise program designed for the individual with dizziness and poor balance secondary to an inner ear problem
Do I need a physician referral to receive therapy treatment?

Most insurance would require a physician referral (a signed physician order/prescription) to cover your therapy visits. Please email the signed order to FauquierPT@fauquierhealth.org or fax it to us at 540.316.2681.

What do I need to know before my first therapy appointment?

Your initial evaluation will take approximately one hour. Please arrive 15 minutes early for your first visit to complete the necessary paperwork. Filling out all of your paperwork before your first appointment will speed up the check-in process. If you wish to do this, please click on and print the relevant paperwork here. Please be sure to download the age-specific patient intake form and fill it out before your arrival.

Co-pay, co-insurance and deductible payments are due at the time service is provided.

PLEASE NOTE: We enforce a strict 24-hour-notice cancellation policy.

How long does each session last?

Generally, your initial visit will last approximately one hour since the therapist will need to evaluate your condition, provide treatment and give home instructions. Following visits may last from 45 minutes to one hour depending on your condition and level of activity.

How often will I need to come?

Most prescriptions and treatment schedules are for two visits per week, but this will vary based on your condition and your needs. After your initial assessment, your therapist will create an individualized treatment plan and will collaborate with you in setting the frequency and duration of your treatment plan.

What do I need to wear for my initial appointment?

Wear comfortable, loose-fitting clothing and a pair of rubber-soled sneakers — something you can move in. Every treatment course begins with a comprehensive evaluation.

What if I am not able to make my scheduled appointment?

24-hour notice is required for cancellations. Please cancel your appointment by calling the office at 540.316.2680. To ensure the best outcomes and reduce the scheduling wait times, we strictly enforce our cancellation and no-call/no-show policy. Frequent cancellations or no-call/no-shows may result in removal from the schedule.
