Advance Directives

An Advance Directive is a written document that tells what a person (declarant) wants or does not want if he/she in the future can't make his/her wishes known about treatment. Fauquier Health, The Virginia Department of Health and the Virginia Department for the Aging provide a booklet to answer all your questions. You can stop by the Fauquier Hospital Admissions Department, near the main entrance, to request a copy or you can find extensive, up-to-date information and printable forms on the Virginia State Bar webpage.
