Online Pre-Registration Instructions

Please have the following available during the pre-registration process:

• Physician's name
• The written order from the referring physician
• Date and time of the procedure, test or surgery
• Type of appointment
• Insurance cards

Most insurance companies do require pre-certification and/or a referral from your primary care physician prior to services. Fauquier Hospital strongly encourages each patient to follow-up with their insurance company to verify that all necessary pre-certifications and referrals have been completed prior to their visit. Failure to obtain pre-certification, physical referral or pre-authorization may result in a lower reimbursement or denial of benefits by the insurance company. Pre-registration allows us to validate all demographic and insurance information prior to your visit. On the day that you arrive, we will simply ask you for a copy of your insurance card and a signed consent form. For your convenience, if you wish, you may print and sign the consent form to bring with you on the day of your appointment.

Please remember to bring the following items on the day of your appointment:

• Physician Order
• Insurance Card(s)
• Personal Identification
